Fatal Crash During Military Operation

A US Air Force KC-135 Stratotanker crashed in western Iraq on March 12, 2026, killing all six crew members aboard during Operation Epic Fury, a military campaign involving cross-regional tensions with Iran. US Central Command confirmed the fatalities March 14 following recovery efforts. The aircraft, tail number 62-3556, went down in friendly airspace with at least one crew member serving with the Ohio Air National Guard.

Investigation Underway

Two KC-135s were operating in the area when the crash occurred, with the second aircraft landing safely, potentially at an Israeli base according to Israeli Ambassador Yechiel Leiter. CENTCOM emphasized the crash was not caused by hostile or friendly fire. Pentagon investigators are examining possible mechanical failure or issues during in-flight refueling operations, given the involvement of two tankers.

Rising Casualties

The incident brings Operation Epic Fury's death toll to at least 13, with approximately 140 injured. Recent losses include three F-15s that crashed over Kuwait March 1 due to friendly fire and casualties from an Iranian drone strike on a Kuwaiti tactical center. The KC-135 fleet, derived from the Boeing 707 and in service since 1957, currently numbers 378 aircraft across the Air Force.
