NTSB: Preventable Errors Doomed Bering Air Flight 445 Over Norton Sound
The National Transportation Safety Board has determined that the crash of Bering Air Flight 445 resulted from a series of preventable failures, according to the agency's final accident report. Investigators found that the Cessna 208B Grand Caravan EX — registration N321BA — suffered an aerodynamic stall and loss of control after the pilot failed to manage airspeed while operating in severe icing conditions over Norton Sound.
Critical Failures Led to the Crash
The aircraft departed Unalakleet for Nome on Feb. 6, 2025, in a significant overweight condition. Its estimated gross takeoff weight of 9,865 pounds exceeded the maximum permitted weight for known or forecast icing by approximately 1,058 pounds — a margin that sharply reduced the pilot's stall buffer in an already demanding environment. When air traffic control advised the crew roughly six minutes before impact that Nome's runway was closed for de-icing, the pilot slowed from 150 knots to approximately 110 knots. After the autopilot disengaged, airspeed rapidly decayed to 70 knots. Improper control inputs then increased the angle of attack beyond the stall point, and the aircraft struck pack ice, killing all 10 people on board.
Industry Implications
The NTSB found no evidence of mechanical failure, characterizing the accident as a human-factors event. The agency has recommended stricter enforcement of weight limits in icing conditions and enhanced training for single-pilot commuter operations in remote regions such as Alaska — safety gaps that investigators say must be addressed to prevent similar tragedies.
