Analysis

On March 30, 1994, approximately 0900 hours mountain standard time (MST), a Grumman-Schweizer G-164B "AgCat", N6754Q, registered to ATR, being operated by Spanish Fork Flying Service, and being flown by Rex Woodhouse, a certificated commercial pilot, nosed over while landing on a road approximately 10 miles south-southwest of Levan, Utah. The aircraft was substantially damaged and the pilot was uninjured. Visual meteorological conditions existed and no flight plan had been filed. The flight, which was engaged in aerial application, was to have been operated in accordance with 14CFR137.

The pilot reported that upon landing he heard a "snap" and the aircraft nosed over. Post crash examination by FAA inspector Jerry E. Roberts revealed that the left main wheel outer bearing (rollers and retainer cups) had broken apart (photograph 01). The outer bearing then separated from the wheel axle. The left brake caliper assembly subsequently released lodging against the left gear strut which restricted rotation of the wheel (photograph 02).
