On May 18, 2004, at 1723 mountain standard time, a Piper PA-18-150, N62712, nosed over during the landing roll at Chandler Municipal Airport, Chandler, Arizona. Chandler Air Service operated the airplane under the provisions of 14 CFR Part 91. The certified flight instructor and pilot receiving instruction were not injured, and the airplane was substantially damaged. Visual meteorological conditions prevailed, and no flight plan had been filed. The local instructional flight originated at Chandler Airport at 1635.

The CFI stated in the Pilot/Operator Accident Report (NTSB Form 6120.1/2) that they remained in the airport traffic pattern the entire flight practicing touch-and-go's.
