On January 20, 2016, about 1910 universal coordinated time, an Air Tractor AT401B, Brazilian registration PT-WFX, was destroyed by collision with a motor vehicle and terrain in Londrina Municipality, State of Parana, Brazil. The Brazilian certificated pilot was seriously injured. Six occupants of the motor vehicle were fatally injured, while two occupants were seriously injured. The flight departed Londrina Airport (SSOK), Londrina Municipality, Brazil, and was destined for Sao Paulo, Brazil.
According to investigators, the airplane was on a ferry flight and experienced a total loss of engine power immediately after departing runway 10. The airplane turned sharply back towards the airport at low altitude, and struck a vehicle travelling on the highway at the approach end of runway 28. The airplane and the vehicle were each destroyed by impact and post-crash fire.
