On June 25, 2016, a Lake LA4 (CC-POK) was involved in an accident near Región De Los Lagos, CI. The accident resulted in 2 fatal injuries, 2 serious injuries. The aircraft sustained substantial damage.

On June 25, 2016, about 2045 coordinated universal time (1645 local time), a Lake L-4-200, amphibious airplane, Chilean registration CC-POK, was substantially damaged when it collided with a powerline during a local flight over the Región de Los Lagos, Chile. The pilot and one passenger were fatally injured; two passengers received serious injuries. The local flight originated from Gamboa Airfield (SCST), Castro, Chile.
