On December 7, 2016, about 1150 UTC, an Industria Aeronautica Neiva S/A EMB 721D, Brazilian registration PT-REI, registered to and operated by a foreign individual, collided with trees then terrain shortly after takeoff. Visual meteorological conditions prevailed at the time and flight plan information is unknown for the non-U.S., non-commercial flight that originated 1 minute earlier from Manaus/Flores Airport, Manaus, Brazil, destined for Novo Aripuana Airport, Aripuana, Brazil. The airplane was destroyed, and the commercial pilot and 5 passengers were fatally injured.

Shortly after takeoff from runway 11, witnesses reported the engine experienced a loss of power and the airplane was observed losing altitude. During a forced landing in a wooded area east of the airport, the airplane impacted trees then came to rest inverted on the ground. One passenger survived, but died while hospitalized shortly after the accident.
