On August 23, 2012, about 2020 universal coordinated time (1520 local time), a Bell 212, Peruvian registration OB-1912-P, operated by Los Andes S.A.C., was substantially damaged when it impacted terrain while approaching a jungle landing site about 19 nautical miles east of Malvinas, Peru. The Peruvian and Ecuadorian pilots, and two passengers, were not injured. One passenger sustained serious injuries. Visual meteorological conditions prevailed for the flight conducted under Peruvian flight regulations.According to Peruvian authorities, about 80 meters from the landing site, the pilot lost directional control and the helicopter impacted the ground and rolled onto its side.
The investigation is under the direction of the Republic of Peru.
