On June 14, 2013, about 0810 universal coordinated time, a Bell 212 helicopter, Brazilian registration PR-HRZ, operated by an oil and gas exploration and production company, was destroyed when it impacted terrain shortly after takeoff from a support base located in Tefe, Amazonas, Brazil. The pilot and a passenger were fatally injured. The flight was destined for the Tefe Airport (SBTF), Amazonas, Brazil.
