On January 29, 2014, about 1340 universal coordinated time, a Cessna 140, Brazilian registration PP-DFW, was substantially damaged during impact with terrain, following a loss of control on approach to Cachoeira do Sul Airport (SSKS), Cachoeira do Sul, Rio Grande do Sul, Brazil. The flight instructor and student pilot were fatally injured. The local training flight had also originated from SSKS.
According to information released by the Aeronautical Accident Prevention and Investigation Center (CENIPA) of Brazil, the airplane was on approach and joining a downwind leg of the airport traffic pattern at SSKS. The flight instructor then reported a loss of elevator control, which was followed by an uncontrolled descent to the ground.
