On August 12, 2014, a Piper PA12, Iceland registration TF-ROD, sustained minor damage following a reported failure of a wooden propeller near Reykjavik, Iceland. The pilot and one passenger were not injured. The personal flight was being operated by a private pilot flying cross country near the southern coast of Iceland.
The Icelandic Transportation Safety Board (ITSB) is investigating the incident. As the State of Manufacture and Design of the airplane and propeller, the NTSB has appointed a US Accredited Representative under the provisions of ICAO Annex 13 to assist the ITSB in its investigation. At the request of the ITSB, the NTSB has agreed to examine the wooden propeller assembly at its laboratory in Washington, DC.
